IMMUNIZATIONS OF STUDENTS


The Principal will notify parents of this requirement at the earliest possible date, so that the necessary plans can be made with the family physician or other medical resources to accomplish this standard prior to a child being admitted to school.

A child shall be exempted from the above immunization requirements if he/she presents evidence from his/her physician that immunization will be detrimental to his/her health. A child shall be excused from immunization for religious reasons, upon the signing of a notarized form by the parent or guardian stating that the child has not been immunized because of religious beliefs.

Any child being admitted to the Auburn School District must present proof of meeting the physical examination and immunization requirements within thirty (30) days of entrance. Failure to comply with this provision may result in exclusion from school for the child.

IMMUNIZATION REQUIREMENTS


If > 7 years, three doses DTP, DTaP, DT or Td with one dose after the fourth birthday.

Tetanus booster required if > ten years since last tetanus containing vaccine [Tetanus/diphtheria (Td) toxoid recommended].

Three doses of TOPV/eIPV with one dose after the fourth birthday; and

One dose of MMR given on or after the first birthday and a second dose of measles vaccine required before entering seventh grade and every grade thereafter, and given no less than one month after the first dose.

Hepatitis B for children born on or after January 1, 1993. Three doses are required with a minimum age for dose three is six months.

If < 7 years, four doses DTP, DTaP, DT with one dose after the fourth birthday;


Medical Exemptions

Medical exemptions are granted to a child if his/her physician thinks that an immunization against a particular disease may be detrimental to the child's health. The exemption must be written on the physician's letterhead and must be signed by a licensed physician. A separate medical exemption is required for each antigen. A specific time frame must be stated for the exemption or for when the situation will be re-evaluated.

Religious Exemptions

Religious exemptions may be granted for all vaccines for an indeterminate length of time. The official New Hampshire Certificate of Religious Exemption form must be signed by the parent/guardian and notarized. All exemptions forms must be maintained with health records.
